Obviously had she/her team not emphasized her smile/mouth so much from 1998-2004, then it wouldn't be a big deal.  But they did.  It's almost impossible for people to not comment about it.

People talk about Janet's nose all the time but despite the work there, her smile is fairly intact.

She looked great at the 2016 VMA; her smile was intact but I guess it's a fickle situation because of the surgery around her mouth.  Her appearance is going to vary just because of that surgery.

But whatever she posts on Instagram should never be criticized like that.  She's not doing an official shoot or appearance.  The idiots commenting negatively on her Instagram should get a life.
