she got an upper lip lift surgery and lip injections 

her real problem now is her skin...whe she forces her smile those weird lines appear and maybe she got her bichat fat pads removed

its either fillers or botox in her cheeks that are paralyzing parts of her face. That causes the Joker smile.

Her lips look fine when they are closed, the smiling is the problem because her lips CANT curve naturally at the sides
