I don't have an account, I just log in with facebook and I'm able to stream music! Before that, I switch my country to United States on Tunnel bear and that's it... I hope it counts in the US

5) Save the .txt file and name it something like "Proxy list"

 

6) Go the Elite Proxy Switcher's window and press Ctrl+O to open the list.

 

7) Let the program test the proxies and select one by double clicking on it. Proxy addresses with "Level 1" written next to them are the best. Use those.
